What's wrong?
The brake pedal assembly is the hinged pedal mechanism the driver presses to slow or stop the vehicle. In 2021-2025 Tlx, Mdx, and Pilot vehicles, a pivot pin holds that pedal in its proper position so pedal movement is transferred cleanly into the brake system. The same pedal position also helps control when the brake lights turn on and off.
The problem is the pivot pin was not secured properly during supplier production. The supplier missed the staking step, the manufacturing step that locks the pin in place by deforming metal around it, after a production move and training gap. A scanning-system malfunction also let the missed step pass through production. With the pin unstaked, the brake pedal can shift out of position.
Owners can see a brake malfunction light, movement in the brake pedal, or an abnormal pedal feel while braking. The brake lights can also stay on even when the brake pedal is not being pressed.

What's the safety risk?
A brake pedal that shifts out of position can keep the driver from applying the brakes as intended, increasing the risk of a crash or injury. Watch for a brake malfunction light, pedal movement, an abnormal pedal feel, or brake lights staying on after the pedal is released. Schedule the inspection soon. Repair is free at any franchised Acura dealer.
What should I do?
- Check your VIN to confirm your 2021-2025 Tlx, Mdx, or Pilot is included in this recall.
- Contact a franchised Acura dealer to schedule the free brake pedal assembly inspection and replacement that fixes the improperly secured brake pedal pivot pin.
- Bring the recall notice if Acura has mailed one. If not, reference recall number 25V391 when you call.
- Drive cautiously until the repair is complete. Leave extra following distance and avoid hard braking when safe to do so.
What happens at the repair
At the dealer, an Acura technician inspects the brake pedal assembly and replaces it if the pivot pin or assembly is not secured correctly. The inspection, replacement parts, and labor are free under the recall. Honda used its general reimbursement plan for this campaign, so documented brake pedal repairs paid before the recall notice can be reviewed for repayment. Bring receipts and repair paperwork to the service desk, or contact Honda customer service if the dealer asks for a claim path.
