On 2018-2020 Mci D4500 and D4505 coaches with the steerable tag axle, the tag axle is the rear support axle that helps carry weight and track behind the front steering axle. A tie rod and locking clamp blocks hold that axle in its forward steer orientation, so the rear of the coach follows the intended path instead of steering off-line.
On affected coaches, the clamp blocks had paint on the clamping surfaces, unmachined fastener surfaces, and fastener contact surfaces that did not sit square to the tie rod clamp surface. That combination lets the clamp fasteners lose torque under load. Once the clamp grip relaxes, the tie rod can slide inside the lock clamps and push the tag axle out of alignment with the front axle.
