What's wrong?
The lug nuts on the 2019 Mazda MAZDA3 clamp each wheel to the wheel hub. The hub is the metal mounting point that spins with the wheel, and the nuts hold the wheel tight against that hub through the hub bolts and flange.
On affected vehicles, a manufacturing process error left a partial gap between the hub bolt head and the hub flange during assembly. Even though the lug nuts were tightened correctly at the plant, that gap lets the joint settle as the vehicle is driven. As the joint settles, the nut loses torque, which is the clamping force that keeps the wheel tight. If enough torque is lost, the lug nuts loosen and a wheel can detach from the vehicle.
Mazda identified rattling noise as the warning sign before a wheel falls off. If you hear rattling from a wheel area, treat it as a sign that the wheel nuts need immediate dealer attention.

What happens at the repair
At the dealer, a Mazda technician tightens the front and rear wheel lug nuts to the correct specification. The recall repair is free, and parts and labor are covered for the lug nut tightening. Out-of-pocket repairs already paid are handled through Mazda's new vehicle warranty, not through a separate reimbursement program, because repairs tied to this defect were covered during the new vehicle warranty period. Ask the service desk how warranty coverage applies if you have prior repair paperwork.
